The service provides temporary/short-term accommodation and support for people who need it, for them to gain or enhance existing skills with a view to moving on to a less intensive supported environment in the community.
The service aims to move tenants on to independent living within six months, although this will be based on the individuals progress.
It is registered as an integrated housing support service with the Care Commission.
13 places in supported accomodation are available in Bloomfield Road and Palmerston Road.
Service User Profile:
The service is provided for people aged 16 years of age or over (70% who are 16 -26):
- whose existing care arrangements have broken down or are at risk of breaking down placing the person at risk
- where there is a high risk of family breakdown or breakdown has occurred
- who are homeless or at risk of homelessness and considered to be a vulnerable person
- whose involvement in work, training or education cannot be maintained
- who have significant health problems (including mental health or substance misuse) with a risk to safety or independence
- where abuse or neglect has occurred or may occur
- whose needs cannot be currently met in independent accommodation in the community
- who might be people leaving care (including Pathways clients), hospital, prison to no fixed abode, clients of Community Mental Health Team, Community Learning Disability Team, Criminal Justice, etc.
- who demonstrate a commitment to engage with the service
- who are eligible for housing and to receive either directly or indirectly public funds and be entitled to reside in the United Kingdom.
The assistance and support provided to individuals is determined by an assessment of the service users' needs through which a care plan designed to meet those needs is drawn up. The care plan may identify counselling needs, the need to address substance misuse or mental health issues, behavioural problems and resettlement needs covering the areas identified below.
- Basic needs (food, shelter & warmth)
- Housing and homelessness
- Family issues (including social networks)
- Lifeskills (cooking, shopping, household tasks, communication with others, etc.)
- Finance (debts, budgeting, maximising income, etc.)
- Health (Well-being as well as issues such as substance misuse and mental health)
- Criminal justice issues
- Improving employability (activities, education, literacy & numeracy, job focus, IT, etc.)
- Social work issues
- Risk factors
It is envisaged that service users will utilise the service for around 6 - 24 months although this may be longer at Bloomfield Road. The service user will at the minimum have an annual review of their needs and at the very latest continuation of funding will be requested and confirmed following the review. Where the individuals needs cannot be met within a housing support service the Care Manager or named worker will work with the Cyrenians to identify a more appropriate service e.g. residential or other service.
