Wernham House provides supported residential and respite care for men and women who are aged 18 and over with alcohol misuse problems and who may have mental and physical health issues including alcohol related brain injury and dementia. There are 13 placements for residential longer term support and 4 respite placements. Referrals should be made by a Single Shared Assessment and an agreed project visit is necessary
The service is provided for individuals :
- whose existing care arrangements have broken down or are at risk of breaking down placing the person at risk
- where there is an imminent threat to person's safety or survival where abuse or neglect has occurred or may occur
- who are homeless or at risk of homelessness and considered to be a vulnerable person
- who have significant health problems (including mental health or substance misuse) with a risk to safety or independence
- whose needs cannot be currently met through supported accommodation or a housing support service
- who demonstrate a commitment to engage with the service
- who are eligible for housing and to receive either directly or indirectly public funds and be entitled to reside in the United Kingdom.
Nursing care is not provided by staff at Wernham who dispense all medication through the Boots Monitored Dosage System. Support is given to address personal hygiene and access medical services. Wernham House in line with The Cyrenians Aims & Values use a person centred approach with each service user having a designated keyworker and co- keyworker. Whilst being able to consume alcohol within the project, all service users are required to address their alcohol misuse and utilise support available. Through one to one keywork a care plan is agreed with achievable goals. They are realistic, sustainable goals and can enable a more optimistic view of life. Regular reviews are held together with named workers from Integrated Alcohol Services. Recovery may take place over a long period of time and at the centre of recovery based care is the uniqueness of the individual.
Respite and initial individual assessment of needs would highlight the issues that would require intensive support and placement into short term respite care. Wernham House would give a secure and safe base with experienced staff and initial support may be to address physical and mental well being. Regular reviews in line with the planned intervention would be agreed with the Integrated Alcohol Services Senior Social Worker and team. The agreed respite is time specific and mandates allowing a free flow of information would be signed with the understood agreement of the service user.
Once improved health and social functioning is realised, realistic goals are agreed to promote reduction in risky behaviour, development of practical skills and improved economic wellbeing. This could include, relapse prevention, budgeting, attending appointments and access to community facilities. An individual support package is agreed with the service user, Integrated Alcohol Services Worker and all relevant agencies involved in the resettlement. Division of responsibilities are clear and understood and put in place by all relevant agencies before the service user leaves respite care. This ensures continued contact and engagement of transitional care to enable appropriate resettlement for the individual.
